What to Know Before Your First Visit

Whether it's your first time at the dentist or just your first time with us, we’re committed to making your experience comfortable, informative, and stress-free. Your initial visit is the beginning of a lasting relationship built on trust, compassion, and high-quality care.
We’ll take the time to get to know you, review your dental and health history, and perform a comprehensive exam. Our goal is to provide personalized care that supports both your immediate needs and long-term oral health.
Medical History Form
To ensure safe, customized treatment, please download and complete our Medical History Form before your first appointment. This helps us understand any current conditions, medications, or allergies that may affect your dental care.
No-Show and Late Cancellation Policy
We understand life happens. If you need to cancel or reschedule, we kindly ask that you give us at least 24 hours’ notice. This allows us to offer that time to another patient in need of care.
Please take a moment to review our office policy:
- 24-Hour Notice is required to cancel or reschedule an appointment.
- No-Shows are recorded when a patient misses an appointment without notice.
- Late Cancellations are those made with less than 24 hours’ notice.
- After First Offense: You’ll receive a courtesy reminder of our policy and assistance with rescheduling.
- After Second Offense: A $50 reservation deposit is required to book future visits (applied to your appointment or forfeited if missed again).
- After Third Offense: Continued missed appointments may result in dismissal from our practice.
Payment Options
For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed.
